Congressionals are usually initiated by a family member contacting their Congressman's local office via phone / email / personal visit. There's no particular form that's used, to my knowledge.
FYI -- I was privy to numerous Congressional inquiries "back in the day" and they almost never worked out the way the Soldier intended. In most cases, the chain of command was able to successfully substantiate the actions / punishment in question. The Congressman/woman (actually their staff) would usually read the COC's response and would basically say "ok roger, got it."
